Welcome to YLA's online member application. We're glad that you are excited about joining but we wanted to take a moment to outline the application process so that you don't forget something along the way.
Fill out the form below
with the most accurate and honest information. Don't feel you need to impress us, you're more impressive than you think, and so you need only worry about being honest.
After you have successfully submitted this online application, an email will be sent to you confirming that we did indeed receive your application. Do not forget to download
PDF version of a parental waiver
you must get signed in order to join YLA.
Print out and have your parent or legal guardian
sign the waiver
and either mail the waiver to the address provided or
bring it in person
to the September 29th Movie Night at the Tiger Woods Learning Center.
